WHAT	Explosion in the material coating room during equipment operation.
WHEN – WHERE – WHO	9:30 AM, May 21, 2025, at a magnet manufacturing plant in Quang Nam province
WHY – HOW	After the machine alerted abnormally high O2 levels, a worker manually opened a valve to inject nitrogen. An explosion occurred 30 seconds later. O2 concentration exceeded safety limits, manual intervention in a high-risk environment may have triggered ignition or uncontrolled reaction.
CONSIQUENCE	12 workers injured, one critically with 60–70% body burns. Structural damage to the workshop; power and fire suppression systems were automatically activated
