WHEN – WHERE – WHO	About 20:30 in Jan 2, 2022 at Covid-19 medical facility in My Tho city – Tien Giang province
WHY – HOW	Crane the oxygen tank inside the medical facility for installation Crane truck was lifting an oxygen tank (~ 10m long) from another truck to inside the medical facility and suddenly flipped sideways, and the tank fell in the ground Cause is under investigation
DAMAGED	No any injured case
LESSONS LEARNED	Crane drivers must have a crane driver’s license Cranes, related equipment such as cables, crane hooks must be within the inspection period Perform hazard identification, risk assessment of crane area and tank installation prior to work
